FC Volunteer Application

I certify that the information provided in this application is true and complete and I acknowledge that this application is not a guarantee of participation and that my participation is "at will" and may be terminated by First Christian Church or myself at any time with or without cause. I understand that, upon approval, I may be subject to a Background Check (National & Local Criminal Background search and National Sex Offender Registry search). I understand that if I am approved to serve as a volunteer with First Christian Church, I will be committing myself to serving others and reflecting Jesus through my actions and attitudes. I understand that I may be required to complete designated training associated with my area of service. I have read First Christian Church' statements of Basic Beliefs and Mission/Vision/Values and agree to do nothing in words or actions that takes away from the ministry of First Christian Church.
Furthermore, I agree to follow and enforce all guidelines and rules set by the leadership of the church. I understand that pictures/videos may be taken of me while participating in activities and that these pictures/videos may be used in publications, promotions, websites, and slide shows.
I KNOWINGLY AND VOLUNTARILY ASSUME ALL RESPONSIBILITY FOR RISKS INVOLVED IN MY PARTICIPATION AS A VOLUNTEER WITH FIRST CHRISTIAN CHURCH.. I AGREE TO RELEASE, INDEMNIFY, AND HOLD HARMLESS FIRST CHRISTIAN CHURCH, ITS MEMBERS, EMPLOYEES, AND ATTORNEYS FROM ANY AND ALL CLAIMS ACTIONS. AND/OR LOSSES FOR BODILY INJURY PROPERTY DAMAGE WRONGFUL DEATH, LOSS OF SERVICE, OR OTHERWISE WHICH MAY OCCUR AS A RESULT OF MY PARTICIPATION.
